The Health Fund for Frederick County Youth
Accepting Grant
Applications
March 18, 2010. The
Community Foundation of Frederick County is accepting proposals for
funding from The Health Fund for Frederick County Youth. The Fund has
up to $20,502 available to award.
The purpose of this fund is to support organizations providing
services to Frederick County youth under the age of 18 years who have
extraordinary and/or life threatening medical needs. Grants may
support purchasing equipment, providing surgery related to birth
defects, and/or paying for other medical expenses not reimbursed or
covered through insurance.
Grant applications may be received at any time during the year.
Applicants may request any amount; multi-year funding requests will
not be granted. Preference shall be given to those applicants that can
spend grant monies before June 30th of each year. This grant will not
be paid to families or individuals, but paid directly to vendors
and/or medical providers. A letter from the current attending
physician/medical practitioner certifying need and diagnosis must be
submitted as part of the application packet. In addition, applicants
must certify they are either a 501(c)(3) nonprofit organization, a
government agency applying for a need not funded by tax revenue, or a
religious organization applying on behalf of a Frederick County youth.

Applications and required documentation may be mailed to the Community
Foundation at 312 East Church Street, Frederick, Maryland, 21701. The
Community Foundation will consider all applications carefully and at
its sole discretion may award all, part or none of the available grant
dollars from The Health Fund for Frederick County Youth. Potential
grantees will be notified of their status within 60 days of submitting
the application package.
The Community Foundation of Frederick County was established in 1986
